To avail this special service :
Beneficiary must send the below documents in soft/scanned copy, to covidclaim@metlife.com.bd by 12:30pm on a working day to be eligible for 3 hours claims decision process. Any claims request received after the cut-off time will be processed (decision making) on the next working day. Scanned or soft copies of the documents must be clear to read.
- Duly filled Individual Claim Form
- Death Certificate and COVID-19 test report of Policyholder
- National ID card/ Passport/driving license/Birth Certificate of policyholder
- National ID card/ Passport/driving license/Birth Certificate of Beneficiary
- Passport/Birth Certificate of Child for Juvenile/Child Policy
- Latest Policy Specification Schedule (PSS) and policy application form
- Scanned copy of a MICR cheque leaf or Bank Statement with Routing Code of Beneficiary
